HOME FOR ALL AFH LLC is a Licensed Adult Family Home (AFH) in Sun Prairie, WI providing 24/7 compassionate, family-style care in a warm, safe home environment. We believe every senior deserves to feel valued, respected, and cared for like family. Located in a quiet, safe neighborhood at 3249 Weybridge Dr, Sun Prairie, WI 53590, our home is designed for comfort and peace of mind. We are a family-owned and locally operated Adult Family Home offering personalized care for 3-4 residents — not a large facility. We treat every resident as our own family. Your loved one will enjoy home-cooked meals, companionship, and daily support in a real home.
SERVICES PROVIDED BY HOME FOR ALL AFH LLC
Meals and Snacks
we will provide three meals, including snack between each meal per state requirements.
Health monitoring.
Home for All AFH LLC will monitor the residents’ health by observing and documenting changes in each condition with immediate action and appropriate referral to their healthcare providers. This will also include notifying their guardian/POA and contacting the placing agency relating any change in the residents’ conditions.
Medical appointment, scheduling, and transportation.
These services will be provided to the residents according to their service agreements and/or individual service plan. We will also make sure that each resident has an annual health exam by their primary care physicians or appropriate provider.
Daily care.
Home for All AFH LLC will provide supervision and assistance with activities of daily living (bathing, dressing, toileting, eating, etc.) per their individual care plan.
Transportation.
The transportation will be provided or arranged according to the service agreement and/or service plan to their employment, recreational, leisure, and other social activities.
Managing a resident’s personal fun
We will provide proper financial management of residents’ personal fun in accordance with their service agreement and keep a log.
Medication Administration
We will provide or assist residents in dispensing their medications according to their individual care plan or service agreement.
Activity program.
We will provide different types of activities that will cater to the needs and preference of each member.
Laundry and housekeeping.
We will provide household items needed and make them accessible to all residents for use. The staff will assist members who are unable to complete these tasks and supervise those who can.
Nursing care service. We will not provide nursing care service to the residents, nor will we pay for the service provided by the home health agency.
Alcohol and tobacco use. Our facility does not allow any smoke and tobacco use in the home.
Pets. Pets are not allowed in the facility with the exception of fish unless it is therapeutic.
